Fail fast, fail early, fail cheap VS. the black hole

Earlier this week I noticed myself putting on an earnest face and using the phrase “fail fast, fail cheap, fail often.” What was I REALLY talking about I asked myself later on. What does it imply?
And anyway, fail fast, cheap and often as opposed to what?
I believe it’s mainly the design process and a commitment to iterate and experiment that makes the difference. (See notes)
